Shaping the future for patients living with diabetes

Our team vision is to ensure that patients living with diabetes are given the best possible advice and care by skilled professionals, who educate and support them to manage their condition.

We want diabetes to be recognised as a priority for primary and secondary care.

Our project will improve patient experience, and health outcomes, through improved education and professional collaboration. We will achieve this by working together; taking time to listen, understand and value different perspectives. We will put our patient’s best interests at the heart of our work and ensure that we involve them, and multidisciplinary teams, to co-produce the new shared pathways.

These new pathways will have more streamlined processes, so that patients can leave and join at the appropriate point, in a seamless way. The mechanics and transition of this should be so smooth that they do not even notice that they are moving between different providers or lead professionals.

The right information and care plans will reach the right people, at the right time.

Patients will feel educated and empowered in managing their condition and know when and why to ask for help.’
